A holder of mutual fund shares, equity, or bonds with a registered mutual funds company<\/strong> can apply to change his\/her registered signature or to provide a new signature in case of non-availability by filling the following affidavit form through the company\u2019s official Registrar and Transfer Agent.<\/strong><\/p>\n\n\n\n

Along with the\nrequest to change\/add a new signature, the applicant will also have\nto provide his\/her consent to the company in written form, agreeing\nto various terms and conditions as mentioned below.<\/p>\n\n\n\n

AFFIDAVIT (FOR CHANGE\nOF SIGNATURE\/NON AVAILABILITY OF SIGNATURE)<\/h3>\n\n\n\n

It has the following 9 parts:<\/strong><\/p>\n\n\n\n

1. Folio Details<\/strong><\/h3>\n\n\n\n

The shareholder will provide details, such as his name, present address, company (mutual funds) name & address, details of the folio, folio number, number of shares, name of the company, certificate number, etc.<\/p>\n\n\n\n

2. Address Declaration<\/strong><\/h3>\n\n\n\n

This is the\ndeclaration that the applicant is presently residing at the\nabove-mentioned address and has not changed his\/her address in the\npast 3 years. Otherwise, he\/she will enclose an attested proof of new\naddress.<\/p>\n\n\n\n

3. Share Declaration<\/strong><\/h3>\n\n\n\n

The shareholder declares that the above shares were purchased by him and mentions the purchase year.<\/p>\n\n\n\n

4. Old Signature Cancellation<\/strong><\/h3>\n\n\n\n

Provides his\/her\nconsent that his\/her current signature recorded with the company\nshould be cancelled as they are no longer valid or have changed.<\/p>\n\n\n\n

5. New Signature Validation<\/strong><\/h3>\n\n\n\n

Provides his\/her\nconsent that his\/her new specimen signature provided along with this\ndocument should be recorded with the company as an effective\nreplacement of his\/her existing\/current signature with the company.<\/p>\n\n\n\n

6. Affirmation of Non-Liability of Company<\/strong><\/h3>\n\n\n\n

The\nshareholder\/applicant affirms that the company or its agents cannot\nbe held responsible for any possible consequences that may occur in\nreaction to this request of changing the signature.<\/p>\n\n\n\n

7. Affirms for\nTransfer Deeds<\/strong><\/h3>\n\n\n\n

The applicant\nconfirms that the transfer Deeds for the sale of securities bearing\nhis\/her new signature should be considered valid.<\/p>\n\n\n\n

8. Identity & Address Proof<\/strong><\/h3>\n\n\n\n

The applicant\nprovides self-attested copies of his\/her identity proof, address\nproof and a recent photo.<\/p>\n\n\n\n
