[Pursuant to subsection (3) of section 72 of the Form SH-14 of<\/strong><\/em> Companies Act, 2013<\/strong> and rule 19(9) of the Companies (Share Capital and Debentures) Rules 2014]<\/em> <\/p>\n\n\n\n

This is the Form No. Sh-14 to be used when you need to cancel or update an existing nomination. You can find here the application format along with the step-by-step procedure for filing the form Sh-14<\/strong> for cancellation or variation of nomination<\/strong>. <\/p>\n\n\n\n

Note:<\/strong> Read the instructions given at the end of this post carefully before you start to fill up this form.<\/p>\n\n\n\n

Sh-14 How to Apply Form No. Sh-14 For Cancellation or Variation of Nomination?<\/h2>\n\n\n\n

The form has the following sections:<\/strong><\/p>\n\n\n\n

Application for Cancellation of Nomination:<\/strong><\/h3>\n\n\n\n

If you want to\ncancel an existing nomination which you made earlier, use the first\nof the two sections in the application. Provide the Company Name &\nAddress to which the application is addressed. Then, enter your name,\nname of the nominee (to be cancelled), and nominee address.<\/p>\n\n\n\n

Application for Variation\/Replacement of Nomination:<\/strong><\/h3>\n\n\n\n

If you want to request to replace an existing nomination in the favor of a new nominee, you need to use the second section of the application. Hereby, you need to provide your name and the name of the old nominee (to be replaced).<\/p>\n\n\n\n

Do not fill up both\nthe sections in any case. If you fill the second section for adding a\nnew nominee as a replacement, the old one will be automatically\ncancelled.<\/p>\n\n\n\n

1) Particulars of the Securities (In Respect of Which Nomination is being Cancelled\/Varied):<\/strong><\/h4>\n\n\n\n

This table will\ncontain details of the securities for which a nomination is being\ncancelled or replaced. Enter the nature\/type of securities, folio\nnumber, no of securities, certificate no, distinctive no, etc.<\/p>\n\n\n\n

2) (a) Particulars of The New Nominee:<\/strong><\/h4>\n\n\n\n

Use this form only\nif you are going to add a new nominee as a replacement to the old\nnominee.<\/p>\n\n\n\n

Fill up the details\nof the new nominee, including their name, father\/mother\/spouse name,\ndate of birth, nationality, phone no, address, email id, and their\nrelationship with the security holder (you).<\/p>\n\n\n\n

(b) In Case New Nominee is A Minor:<\/strong><\/h4>\n\n\n\n

If you\u2019re adding a\nnew nominee who happens to be a minor (below 18 years age), then use\nthis form to provide the details of the guardian.<\/p>\n\n\n\n

3) Particulars of Nominee in Case Minor Nominee Dies Before Attaining Age Of Majority:<\/strong><\/h4>\n\n\n\n

If you are adding a\nnew nominee who is a minor, you also need to add an alternative\nnominee who will become effective in case if the minor nominee dies\nbefore attaining the Age of Majority (18 years).<\/p>\n\n\n\n

Provide the name,\nfather\/mother\/spouse name and other details of the alternate nominee.<\/p>\n\n\n\n

Declaration:<\/strong><\/h3>\n\n\n\n

The last table will contain the details of policy (security) holders, including their name\/s, signature as well as the name, address & signature of the witness.<\/p>\n\n\n\n
