Pursuant to\nsection 72 of the Companies Act, 2013 and rule 19(1) of the Companies\n(Share Capital and Debentures) Rules 2014]<\/em><\/p>\n\n\n\n

This nomination form<\/strong> is to be used by an existing individual, who holds the shares singly or jointly in the Company and wants to assign an official nominee to their investment folio.<\/p>\n\n\n\n

Note:<\/strong> Read the instructions given at the end of this post carefully before you start to fill up this form.<\/p>\n\n\n\n

Here\u2019s how to fill and process the Nominee form.<\/strong><\/p>\n\n\n\n

How to File Form No. SH-13 for Assigning a Nominee to Your Portfolio?<\/strong><\/h2>\n\n\n\n

Application:<\/strong><\/h3>\n\n\n\n

The name of the Trading company along with address & pin code to which this application is addressed. The applicant\u2019s name, address and confirmation to be provided in this column.<\/p>\n\n\n\n

\"SH-13<\/a><\/figure>\n\n\n\n

1) Particulars of The Securities (In Respect Of Which Nomination Is Being Made):<\/strong><\/h4>\n\n\n\n

Details of the\nsecurities, including nature\/type, folio no, the number of\nsecurities, certificate no, distinctive numbers, etc.<\/p>\n\n\n\n

\"SH-13<\/a><\/figure>\n\n\n\n

2) Particulars of Nominee\/s:<\/strong><\/h4>\n\n\n\n

Provide the details\nof the nominee\/s, including the name, father\u2019s\/mother\u2019s\/spouse\nname, date of birth, occupation, nationality, address, email id,\nphone number, and their relationship with the security holder.<\/p>\n\n\n\n

3) In Case Nominee Is A Minor:<\/strong><\/h4>\n\n\n\n

Note: Use this table\nonly in case if the Nominee is a minor (below 18 years).<\/p>\n\n\n\n

Furnish the Date of\nBirth of the nominee, date of attaining majority (18th birthday), the\nname of the guardian, and address of the guardian.<\/p>\n\n\n\n

4) Particulars of Nominee In Case Minor Nominee Dies Before Attaining Age of Majority:<\/strong><\/h4>\n\n\n\n

Note: This table is\nto be used only in case if the Nominee is a minor (below 18 years).<\/p>\n\n\n\n

The applicant needs\nto provide the details of the second (alternative) nominee in case if\na minor nominee dies before attaining the age of majority. This table\nwill contain details similar to table 2 but for the second nominee\nand also their relationship with the Minor nominee.<\/p>\n\n\n\n

Declaration:<\/strong><\/h3>\n\n\n\n

The applicant\n(security holder) is required to provide his\/her name and signature,\nalong with the name, address and signature of the witness, confirming\nthe information provided by them in the application form.<\/p>\n\n\n\n
