The application for the same must be filled in the format prescribed below and be sent to the registered address of the <a href=\"https:\/\/rta.saginfotech.com\">Registrar and Share Transfer Agent<\/a>.<\/p>\n\n\n\n<p>The applicant must\nclearly mention the old name (which he wants to change) and the new\nname (to which he wants to change), along with the reason to change\nthe name. They must also provide supporting documents in the\nattachment.<\/p>\n\n\n\n<p><strong>How to Fill Out a Request Letter for a Name Change?<\/strong><\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>The \u201cChange of Name\u201d Application Form has the following four parts:<\/strong><\/h3>\n\n\n\n<h4 class=\"wp-block-heading\">1. Details of The Share Certificates<\/h4>\n\n\n\n<p>Provide details of\nall the share certificates owned by you in the old name, including\nthe folio number, distinctive numbers, share certificate number, and\nno. of shares, for each folio.
