The completed form, along with supporting documents, must be sent to the company\u2019s official <strong>Registrar &amp; Transfer Agent<\/strong>.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">The format of \u201cRequest Letter for Change of Address\u201d is given below:<\/h3>\n\n\n\n<p><strong>The applicant is required to furnish the following details in their application:<\/strong><\/p>\n\n\n\n<ol>\n<li>Unit\n\t<\/li>\n\n\n\n<li>Folio number\n\t<\/li>\n\n\n\n<li>Shareholder\u2019s\n\tname\n\t<\/li>\n\n\n\n<li>Company name <\/li>\n\n\n\n<li>Old address\n\t<\/li>\n\n\n\n<li>New address\n\t<\/li>\n\n\n\n<li>Proof of New\n\taddress\n\t<\/li>\n\n\n\n<li>Shareholder\/s\n\tname and signature\n\t<\/li>\n\n\n\n<li>Date &amp;\n\tPlace\n\t<\/li>\n\n\n\n<li>Address proof\n\tcopy\n<\/li>\n<\/ol>\n\n\n\n<p>The applicant is required to provide correct address proof showing the new address, not the old one. The address proof copy must be attached to the application and sent to the concerned party, <strong>Registrar and Transfer Agent<\/strong>.
